Scope of Outreach: Our communication is limited to the administration of your care, including but not limited to: automated scheduling confirmations, crucial pre-treatment safety protocols, direct follow-up inquiries by our clinical staff, and internal practice announcements regarding new medical technologies or seasonal events.
Compliance Standards: Consent is elective and is not required for the provision of medical services. Data and message rates established by your mobile provider may apply. You maintain the right to revoke this authorization at any moment by responding STOP to any digital transmission. 3. Financial Management: We disclose necessary data to facilitate payment for services. This may include verifying financing eligibility with third-party providers or managing disputes through financial institutions.
B. Mandatory & Legal Disclosures
Under federal law, Elite MedAesthetic may be compelled to release your PHI without your consent in the following scenarios:
- Public Health Oversight: Reporting communicable diseases, adverse reactions to medical products, or participating in public health investigations.
- Legal Compulsion: In direct response to a valid court order, administrative subpoena, or other legally sanctioned discovery request.
- Safety Intervention: When disclosure is necessary to prevent an imminent and serious threat to the health and safety of the patient or the public.
- Oversight Agencies: To health oversight agencies for activities authorized by law, such as audits and inspections of medical facilities.
- Law Enforcement: To identify or locate a missing person, or to report a crime occurring within our clinical premises.
C. Your Individual Rights & Protections
As the subject of this information, you are granted specific rights under federal law:
- Right of Access: You may petition for a copy of your clinical and financial records in either physical or electronic format.
- Right to Correct: You may request an amendment to any portion of your PHI that you believe to be factually inaccurate.
- Right to Restriction: You may request that we limit how we share your information for treatment or operations. Note: While we are not always required to agree, we must comply if you pay for a treatment entirely out-of-pocket and request that we not share that data with a health insurer.
- Right to Disclosure Accounting: You may request a chronological log of all instances where your PHI was disclosed for reasons other than routine treatment, payment, or operations.
